Online Registration for Leagues, Camps, Clinics, and Tournaments
All participants in individual programs or team leagues, tournaments, and other activities must sign a
roster form prior to using the Family Sports Center. Parents must also sign the roster form for youth prior
to playing. We use the same form for both individual and team programs. Individuals need only fill out a three
line section at the bottom of the front page of the wavier form. Teams must fill out the top section and each team member
fills out a three line section at the bottom or on page two.
Click below to print out form. Set printer options to "LANDSCAPE." Otherwise the form will not
fit. Sometimes the document flashes on the screen. You will find it open at the bottom as an "Excel" button. Click
it.
