All participants in individual programs or team leagues, tournaments, and other activities must sign a roster
form prior to using the Family Sports Center. Parents must also sign the roster form for youth prior to playing.
We use the same form for both individual and team programs. Individuals need only fill out a three line section at the
bottom of the front page of the wavier form. Teams must fill out the top section and each team member fills out a three
line section at the bottom or on page two.

A tournament at the end ( Adult and High School must Qualify)

Leagues Only $475 per Adult team, $400 per High School team, U6:$250, U8-U10-$300 and U12-U14:$425
Sign up and pay for two sessions and receive a $100 discount on your combined league
fees! (High School and Adult leagues only)
Teams can have up to 14 players. 6v6 including goalie (U14
- Adult leagues) Equipment needed to play: Shin-guards(required High School and younger), tennis shoes, and uniforms.
Uniforms can be same colored shirts.
Individuals can
join a Blue Chip House Team.
The cost for an individual is $45 and includes a Blue Chip Soccer
